Self-management workshops for people living with a chronic condition and/or chronic pain, diabetes education, their caregivers and those at-risk for developing a chronic condition * peer-led * coordinates Choices and Changes and Brief Action Planning Workshops for health care providers * self-management skills workshop * training for workshop facilitators * regional Communities of Practice for Self-management programming
