Description of Services:

Supports for people living with prediabetes or diabetes * one-one-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurses, dietitian, and clerical support on staff

glucose screening clinics
nutritional counselling
foot care/chiropody services

Diabetes education helps patients develop a positive approach to diabetes, set their own goals, produce a personalized meal plan and reduce or prevent complications of diabetes
