Description of Services:
Supports for people living with prediabetes or Type 2 diabetes * one-one-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse, dietitian, diabetes educator and endocrinologist on staff
