Description of Services:
Social program to assist eligible adults aged 60 years and older to remain in their own homes as long as possible
- nutritious snacks
- early detection of potential crisis situations
- health promotion
- social connections
- support for caregiver
- social, activity and educational component
- caregivers may also attend
