Description of Services:
Outpatient program providing comprehensive assessment, treatment, planning, advocacy, referral and education for older adults with cognitive disorders such as dementia, late onset mental health needs, and younger adults with suspected progressive dementia. Services include:
  • comprehensive assessment completed by a registered nurse, social worker, occupational therapist, neuropsychologist, or geriatric psychiatrist
  • treatment recommendations and personalized care planning based on the client's needs and goals
  • collaboration with the client's care team, including primary care, long-term care staff, and community agencies
  • behavioral support and strategies to help manage responsive behaviours
  • education and coaching for care partners and care teams
  • referrals to appropriate health and community services
  • advocacy to support the client's needs and ensure coordinated, person-centred care
  • encouragement for family and care partner involvement

Please note that this program does not typically provide long-term follow-up; the focus is on assessment, behavioural support, and linking individuals to appropriate community supports
