Description of Services:

A multidisciplinary team that provides holistic person-centred palliative care to people from early diagnosis (optimally PPS of 100% - 70%) through to bereavement * care includes disease management, physical, psychological, social, spiritual, practical, end of life care/death management, loss and grief support
