Description of Services:
Supports for people living with diabetes, prediabetes, type 2 diabetes and gestational diabetes * healthy eating, exercise, and other lifestyle choices * dietitian, diabetes educator, social worker, chiropodist and pharmacist on staff
  • non-insulin and insulin injections
  • individual and group counseling
  • foot screening and assessments
  • oral hypoglycemic agents
  • lifestyle management
