Physician referral

Patients 16 years of age or older, medically stable with a clear diagnosis and care plan for recent or active acute issues as well as specific, measurable, realistic goals. Patient must be medically well enough and motivated to participate in from low to moderate intensity therapy progressing from a minimum of 15-30 minutes once per day to 1-2 hours per day, 5 days per week * additional criteria as determined by Providence Care
