Description of Services:

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * core services including health and wellness care, pharmacist services, immunization, allergy and routine injections * team consists of physicians, nurse practitioner, registered practical nurse, registered nurse, social workers, dietitian, pharmacist, psychologist and respiratory therapist
