Description of Services:

Supports for Adults living with prediabetes or diabetes

Education Clinics:

certified diabetes educators, registered dieticians and nurses who work in cooperation with physicians, pharmacists and family members
individual and group appointments
referrals and monthly telehealth sessions with specialists for advanced care

Education on:

diabetes self-management
healthy living and eating
hyperglycemia (high blood sugar)
hypoglycemia (low blood sugar)
common emotions
physical activity
medications
