Description of Services:
Diabetes prevention/management education program
  • group and individual counselling
  • foot care nurse
  • home visits available for clients residing in the community
  • monthly diabetes case management clinics with an endocrinologist
  • community education sessions in pre-diabetes, high risk diabetes and diabetes self-management
