Application
Referrals are accepted from anyone, including self-referrals. Referrals are through The Access Point. Please call or fill out the application form online. Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.

Supportive housing for men and women living with HIV/AIDS and mental health and/or problematic substance use. High support site provide personal support and 24/7 residential support to adults of all ages living with HIV and/or transitioning genders living with mental health challenges and/or problematic substance use..
