All referrals need to be signed by a physician along with their billing numbers

Eligibility / Target Population
Residents of Ottawa 65 years and older with mental health problems or suffering from dementia who are not in a long-term care home, and persons less than 65 with Alzheimer's dementia or Frontotemporal Dementia complicated by behavioral or psychological symptoms

Set fees, Covered by the Ontario Health Card

Description of Services:
Provides comprehensive geriatric mental health assessments to provide diagnosis and treatment options for older adults (65 years and over) living in the community with new mental health concerns, and/or chronic mental illness complicated by age related functional decline, and/or living with dementia and associated behaviour change

Outreach services provide in-home assessment, short-term case management and treatment to optimize quality of life and recover

Interdisciplinary team includes: Geriatric psychiatrists * registered nurses * occupational therapists * social workers * psycho geriatric resource consultant
