Eligibility / Target Population
Individuals 65 years and older must be:
* willing and able to participate in a group setting two mornings a week for 7-8 weeks
* able to manage their own continence
* medically stable enough to participate in an exercise program
* in need of the services of at least two of the disciplines available

Description of Services:
Group programs for seniors who require interdisciplinary professional care with a focus on fall prevention and cognition * individual and group rehabilitation * team includes: registered nurse , physiotherapist, occupational therapist and mental health counsellor * on discharge, clients are referred back to their primary care provider.
Promotes healthy living and the achievement and maintenance of a level of independence * physical and cognitive exercises * socialization * support and education * addresses medical, functional and social issues
