Description of Services:
Coordinated network of specialized assessment, treatment and rehabilitation services for elderly persons whose well being, independence, or functional ability is threatened by multiple health or social problems * hosted by the Ottawa Hospital and a product of the combined efforts of many hospital and community partners

The services include:
  • Geriatric Assessment Outreach Teams: Provides a multidimensional screening assessment for elderly people in their own homes for the purposes of problem identification and triaging to other Regional Geriatric Programs or community resources * The East Outreach team covers the region of Ottawa east of Bronson Ave and the West Outreach team covers the region west of Bronson Ave * referrals are accepted from health professionals, prospective clients, or caregivers, however family physicians must be contacted before a referral will be accepted.

  • Geriatric Day Hospitals: hospital-based programs that provide multidisciplinary team assessment, diagnostic, therapeutic and rehabilitative services on an out-patient basis * Physician referral is required.
