Description of Services:

Community based GAIN clinic provides comprehensive geriatric assessments and care planning and delivery to address the complex needs of many older adults. The inter-professional teams work with older people and their families to develop care plans that support frail seniors with complex health issues so they can remain living at home * Staff include:

Nurse Practitioners
Care Coordinators
Occupational Therapists
Social Workers
Pharmacists
Personal Support Workers
Geriatrician and physician support
Administrative team members
