Primary care outreach program to Seniors aged 65 years and over * Available to residents in Lanark Highlands, Carleton Place and Mississippi Mills

The team visits seniors in their homes, in the community, and in the office * helps seniors to maintain their health, community connection and safety at home

The team includes a registered nurse and a community health worker who collaborate with other health care staff and community agencies

Services Offered include:  

• Help with medication review
• Help with forms
• Accompaniment to medical appointments as appropriate
• Links to supportive community and social programs
• Chronic illness management
• Help accessing housing support services
