Description of Services:
Registered Nurses (RNs) with extensive knowledge and experience assist patients and families as their care manager for both acute situations and chronic conditions. Holistic approach may include:
- community support, nursing, palliative care, accompaniment to medical appointments
- oversight of treatments and procedures
- communication with physicians and other providers
- assistance with access to community resources
- research diagnoses and treatment options
- investigation of alternatives based on patient's needs
