Ontario Health atHome patients are referred by a care coordinator.
Eligibility / Target Population
Ontario Health atHome patients are assessed by a care coordinator to determine eligibility. A Paramed supervisor will assess for eligibility for private pay clients.
Fees
No fee for Ontario Health atHome patients. Call the office for current fee schedule for private pay clients.

Description of Services:
Homemakers and Personal Support Workers are available to provide assistance with daily living, personal care, meal preparation, homemaking services and in-home respite services.
