Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

  • Counselling Services Mental Health and Addiction counsellors are part of the health team and work in cooperation with other health providers (doctors, nurse practitioners, diabetes educators, dietitians, etc.) to support whole health.  Counselling services available to registered clients for:

  • Diabetes Education Programs Supports for people living with type 1 and 2 diabetes, gestational diabetes, prediabetes or those at risk of developing diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian, and foot care nurse on staff * services offered virtually or by telephone

  • Geriatric Assessment and Intervention Network (GAIN) A network of coordinated healthcare services and partners working together to strengthen the supports and capacity of frail older adults. The GAIN program is for frail older adults living at home or in a retirement residence with multiple complex medical problems, including one or more of the following:
    • Cognitive impairment
    • Falls or risk of falls
    • Incontinence
    • Decreased function
    • Impaired mobility
    • Multiple medications
