Description of Services:

Helps clients remain in their own homes for as long as safely possible while deferring the need for hospital admission or long-term care * services are offered to seniors who reside at home, but need the availability of additional unscheduled visits * services include:

personal care (dressing, bathing, personal hygiene, medication assistance, meal preparation)
security checks
light housekeeping
care coordination
