Anyone can make a referral to Ontario Health atHome, the client, a family member, caregiver, friend, physician or other health care professional * call to make a referral, for more information or to request a referral form

Eligibility / Target Population

People who are recovering from illness or injury, and for those who are living with chronic illness or disability * must have an Ontario Health Card for home care services

None for placement services or in-home services

Health care services offered at home, at school, and in the community, as well as access to supported living programs and long-term care * care coordinators work with people in the community or leaving the hospital to determine what help is needed and arrange for care
