Application
Service can be requested by individuals, family members, friends or any health care personnel * all applicants will be screened by the Program Coordinator to determine their current ability to shop for groceries, safely prepare their own meals or have someone available to assist them with their meal needs
Eligibility / Target Population
Seniors 55 years of age or older, adults 18 years and older who are chronically ill, adults living with a disability, adults convalescing from surgery or illness, adults undergoing medical treatment
