Self-referrals accepted * when possible, the individual being referred must agree to access the services * referrals from agencies and other professionals must be in writing and include a completed referral form; mental health professionals (psychiatrists, social workers, psychologists etc.) must include supporting documentation * form for non-urgent referrals available on website

Individuals 65 years and older who have a mental health and/or cognitive problem, memory or mood issues, or need counseling services
