Description of Services:
Provides screening, diagnosis, and management of cognitive changes in the aging population * team consisting of health care navigators, nurse practitioners, registered nurses. social workers and physicians, assesses the causes of memory changes and provides recommendations to improve memory function * caregiver support and education also provided.
