Physician, hospital or self-referral by phone, fax, e-mail. All written and phone referrals should be directed to Central Intake. In person referrals may be directed to the Haliburton Office.

Description of Services:

Care coordinators are responsible for assessing eligibility, determining service needs, developing service plans, authorizing provision of services, coordinating the delivery of multiple services and monitoring ongoing service needs for in-home and school support services. They also assist the client in planning for discharge when services are no longer required. Care coordinators are responsible for assisting ineligible clients to explore other options or services and making appropriate referrals to other services.

personal support
nursing
physiotherapy
occupational therapy
speech-language therapy
social work
dietetic services
case management/coordination
medical supplies and dressings
hospital sickroom equipment
laboratory and diagnostic services
determination for drug coverage under the Ontario Drug Benefit (ODB) Plan.
In-home respite care services are available based upon eligibility
