Description of Services:
Provide individual and group services to patients with Type 1, Type 2 and gestational diabetes.

Services include:
  • assessment
  • nutrition counselling
  • insulin and glucometer instruction
  • pump therapy
  • certified insulin pump program center
  • self-management information and education
