Description of Services:
Access to supportive care at home, at school or in the community * care coordinators work with people in the community or leaving the hospital to determine the help needed and organize care * care is provided in a community setting such as at home, school, residential facilities or community clinics * provides information on other community services and recommends if necessary

Services available to eligible patients:
  • medical care
  • nurse practitioner
  • personal support
  • physiotherapy and occupational therapy
  • speech therapy
  • social work
  • food and nutrition advice (dieticians)
  • palliative care
  • medical products and equipment
  • access to long-term care homes
  • access to supportive care (adult day program, supportive housing, retirement homes and assisted living)
  • auxiliary school health services
  • specialized services for people with specific health problems
  • referral to other community supports

Other specialized services offered to patients include:
  • Health Care Access - helps people find a family doctor
  • Home at Last - special program to help patients leaving the hospital to settle into their homes
  • School mental health and addictions programs - mental health nurses work within the school
  • access to assistance with daily living and supported housing
  • access to respite care and convalescent care (short stays and adult day programs)access to long-term care homes, and complex rehabilitation and continuing care beds
