First Link Navigation services to help guide newly diagnosed persons and their care partners and families through the healthcare system, linking to services and supports
One-on-one counselling
Home visits, office visits or phone visits offered
Group supports for persons with dementia and their care partners (including family and friends) * support groups offered in most Durham Region municipalities at different times of day to accommodate varying schedules
